Job Title: Home Health Admission RN Nurse

Reports To: Director of Nursing / Clinical Supervisor

Position Summary:

We are seeking a compassionate, detail-oriented, and experienced Registered Nurse (RN) to join our home health team as an Admission Nurse. The Admission RN is responsible for conducting initial assessments of new patients, initiating the plan of care, and ensuring all documentation and compliance requirements are met. This role is critical in setting the foundation for the patient’s home health care experience and achieving optimal clinical outcomes.

Key Responsibilities:

  • Conduct comprehensive nursing assessments in the patient’s home environment, including physical, functional, psychosocial, and environmental evaluations.

  • Initiate the Plan of Care (POC) and complete the OASIS Start of Care (SOC) documentation accurately and timely in accordance with agency policies and CMS guidelines.

  • Collaborate with the interdisciplinary team, including physicians, therapists, and home health aides, to ensure coordinated and effective patient care.

  • Educate patients and caregivers on disease processes, medications, safety, and the home health plan of care.

  • Identify patient needs and make appropriate referrals for additional services such as PT, OT, MSW, and HHA.

  • Verify and reconcile medications during the initial visit.

  • Communicate and report patient status and any changes in condition to the physician and clinical team promptly.

  • Maintain compliance with all federal, state, and local regulations, as well as agency policies and procedures.

  • Provide skilled nursing services as needed during the SOC visit.

Qualifications:

  • Licensed Registered Nurse (RN) in the state of Massachusetts.

  • Minimum of 1–2 years of clinical nursing experience, preferably in home health, acute care, or skilled nursing.

  • Proficiency with OASIS documentation and electronic medical record (EMR) systems (e.g., Finale, Axxess, WellSky, HCHB, etc.).

  • Strong clinical judgment, critical thinking, and organizational skills.

  • Ability to work independently and manage time effectively.

  • Excellent communication and interpersonal skills.

  • Reliable transportation and valid driver’s license.

Preferred Qualifications:

  • Prior experience as an admission nurse in a Medicare-certified home health agency.

  • Knowledge of CMS regulations and conditions of participation.

  • Bilingual skills are a plus.

Working Conditions:

  • Patient homes and community settings.

  • Must be able to travel to patient homes within assigned territory.
